Arriving for Your Appointment
When you arrive for your appointment please call us at 513-914-6062 or text us within the Symplast patient app (curbside check-in) to notify us that you have arrived. Please remain in your car until we contact you that the room has been fully sanitized. All patients and guests of Salons by JC are required to wear a mask while in the building and should not linger in the hallways. Thank you for helping us keep all individuals safe during this time.
COVID-19 Screening
Have you or a family member experienced the following symptoms: fever & chills, new or worsening cough, difficultly breathing, sustained loss of smell, taste or appetite, sore throat, vomiting, diarrhea, or body aches?
Have you or a family member been exposed to anyone exhibiting the above symptoms or knowingly has COVID-19?
Have you tested positive for COVID-19 or are awaiting results?
